ADULT CARE COORDINATION
Adult Care Coordination is a service offered to persons receiving Medicaid, Managed Medicaid or Medicaid/Medicare.
The goal of Care Coordination is to improve the overall health and well being of the participant by connecting them with needed services.
Eligibility requires two or more chronic health conditions or one of the following:
CHILDREN'S CARE COORDINATION
Children's Care Coordination is a service offered to children's ages 5 to 21 receiving Medicaid that need additional support to meet their medical and/or behavioral health needs. The goal of Children's Care Coordination is to improve the overall health and well being of the participant by connecting them with needed services. Eligibility requires two or more mental health conditions one of which can be a medical condition/diagnosis.
