CHILDREN'S CARE COORDINATION SERVICES
A voluntary service where the identified child is matched with a professional Care Coordinator who provides assistance and resources to meet the individual needs for success in home, school and community efforts.
Together we will set meaningful health and life goals, access the services and support you deserve and navigate systems that can feel overwhelming! We'll understand your child's unique needs and develop a plan tailored to your family and support a brighter, more balance future!
Please note: Available for children ages 5 years to 21 years, with at least two or more qualifying mental health diagnosis and active Medicaid.

Services for our children include:
- Assistance with linking to a Primary Care Physician and/or Dentist
- Assistance with scheduling school, medical, therapy and mental health appointments
- Assistance with arranging reliable Medicaid transportation
- Provider outreach
- Ongoing collaboration planning with therapists
- Support and advocate in School with IEP and/or 504 plans
- Psychotherapy education
- Crisis Support
- Community Services and benefits
